2019 Annual Physical Examination of DepEd Employees

Pursuant to DepEd Memorandum No. 22, s. 2015 entitled Annual Physical Examination of DepEd Employees (Teaching and Non-Teaching Personnel), as a manifestation of concern by this office on the health status of its employees, all teaching and non-teaching are required to undergo Annual Physical Examination (APE) including Dental Examination in compliance with the Civil Service Commission Memorandum Circular No. 17, s. 1989.

ANNUAL PHYSICAL EXAMINATION OF DEPED EMPLOYEES (TEACHING AND NON-TEACHING PERSONNEL)

In this regard, all teaching and non-teaching personnel are required to bring their CS form 86 on their scheduled date and present it to School Health and Nutrition Unit to get a copy of the Medical Certificate and laboratory request for the following:

  • Chest X-ray Result except for pregnant women (validity is within 6 months before the respective scheduled date)
  • Urinalysis (with validity within the week prior to the date of schedule)

Further, Medical certificates must only be signed by any Government Medical Doctor.

The teacher’s annual health clearance shall be based on the filled out and duly accomplished CS Form 86 and Medical Certificate (Health Examination Record).

Additional Notes:

Teachers’ Annual Health Examination Form 86 may be filled out by the DepEd Medical Officer, Tamang Serbisyong Kalusugang Pampamilya (TSEKAP) Service Provider Medical Officer or any government-licensed physician. All DepEd teaching and non-teaching personnel are encouraged to avail of the TSEKAP formerly known as the Primary Health Care Benefit 1 (PCB1) Package.

To avail of the TSEKAP services, teachers must visit their assigned service provider for enlistment. Profiling of medical history will then be done to determine the needed physical examination. All concerned may refer to DepEd Memorandum No. 30, s. 2014 entitled Expansion of PhilHealth Primary Care Benefit 1 (PCB1) Package to cover Personnel of the Department of Education.
