DepEd Requirements for Certification of Last Payment, Step Increment and Salary Adjustment

Attached is DepEd National Capital Region MEMORANDUM No. NCR 237, s. 2019 dated November 5, 2019 re: Reiteration on the Checklist of Requirements for Certification of Last Payment (CLP), Step Increment and Salary Adjustment (Promotion and Reclassification), the contents of which are self-explanatory, for information and strict compliance.

Particular attention is invited to paragraphs 1 and 2.

Immediate and wide dissemination of this Memorandum is desired.

Reiteration on the Checklist of Requirements for Certification of Last Payment (CLP), Step Increment and Salary Adjustment (Promotion and Reclassification)

To facilitate the issuance of Certificate of Last Payment for the retired, resigned, transferred or separated from the service, and requests for Step-Increment and Salary Adjustment (Promotion or Reclassification) to the payroll system, this Office reiterates the submission of the complete documentary requirements to be submitted in the RPSU, to wit:

CERTIFICATE OF LAST PAYMENT

a. Indorsement of the Schools Division Superintendent

b. Division Clearance (Signed by the Principal/Supervisor/OIC)

c. Latest Service Records

d. Daily Time Record (DTR); and

e. Approved Leave with or without pay (Form-6)

STEP INCREMENT

a. Indorsement of the Schools Division Superintendent

b. Notice of Step Increment (NOSI)

c. Latest Service Records

SALARY ADJUSTMENT (PROMOTION OR RECLASSIFICATION)

a. Indorsement of the Schools Division Superintendent

b. Notice of Salary Adjustment (NOSA) Appointment

c. Updated Plantilia

Incomplete requirements will be returned without appropriate action.
