DepEd Documentary Requirements for Monetary Claims

TeacherPH is hereby reiterating the basic documentary requirements for Salary Claim and Other Monetary Claims to wit:

Documentary Requirements for FIRST SALARY CLAIM OF NEWLY HIRED EMPLOYEES:

In 1 folder with 2 copies each & all photocopies must be authenticated by the Administrative Officer

  • Letter request with contact number
  • Appointment (photocopy)
  • Certificate of Assumption to Duty
  • Advice Order/Assignment Order
  • Form 7/Monthly Payroll Worksheet & Report of Service (all copies should be original)
  • Form 48/Daily Time Record (DTR) (all copies should be original)
  • Statement of Assets and Liabilities (SALN)
  • Oath of Service
  • BIR Form 1902/2305 (duly stamped by BIR)
  • DBP ATM Account Number
  • Pag-ibig Number
  • Philhealth Member Data Record (MDR)
  • GSIS Member’s Information Sheet
  • Additional Requirements for TRANSFEREES (from one government office to another)
  • Clearance from money, property and legal accountabilities from the previous office
  • Certificate of Last Payment
  • Official Receipt (if there’s an overpayment)
  • BIR Form 2316
  • GSIS BP Number
  • Certificate of Available Leave Credits
  • Service Record

Documentary Requirements for UNPAID SALARY OF NEWLY HIRED EMPLOYEES

In 2 original copies each

  • INTEGRATED IN THE REGULAR PAYROLL (succeeding claims)
  • Letter request with contact number
  • Form 7/Monthly Payroll Worksheet & Report of Service (for the applicable month)
  • Form 48/Daily Time Record (DTR) (for the applicable month)
  • Approved Form 6 (If any)

Documentary Requirements for UNPAID SALARY OF REGULAR EMPLOYEES WHO WERE CUT-OFF FROM THE REGULAR PAYROLL

In 2 folders with 2 copies each (where 1 folder is for salary claim & 1 folder for re-integration to RPSU) & all photocopies must be authenticated by the Administrative Officer.

  • Letter request with contact number
  • Form 7/Monthly Payroll Worksheet & Report of Service – all copies should be original (for the applicable period)
  • Form 48/Daily Time Record (DTR) – all copies should be original (for the applicable period)
  • Approved Form 6
  • S.O. maternity/personal leave
  • S.O. reinstatement
  • Certificate of Live Birth (for maternity)
  • Medical Certificate(CS Form 41)
  • Teacher’s Clearance
  • Last pay slip received

Documentary Requirements for Substitute Teacher’s SALARY CLAIM:

In 1 folder with 2 copies each (Photocopies must be authenticated by the Administrative Officer)

  • Letter request with contact number
  • Appointment (photocopy)
  • Certificate of Assumption to Duty
  • Form 7/Monthly Payroll Worksheet & Report of Service (all copies should be original)
  • Form 48/Daily Time Record (DTR) (all copies should be original)
  • Statement of Assets and Liabilities (SALN)
  • Oath of Service
  • School Clearance
  • DBP ATM Account Number
  • Pag-ibig Number
  • GSIS Member’s Information Sheet

Documentary Requirements for UNPAID SALARY OF REGULAR EMPLOYEES WHO WERE FROM MATERNITY/PERSONAL LEAVE BUT NOT YET INTEGRATED TO THE REGIONAL PAYROLL

In 1 folder with 2 copies each & all photocopies must be authenticated by the Administrative Officer.

  • Letter request with contact number
  • Form 7/Monthly Payroll Worksheet & Report of Service – all copies should be original (for the applicable period)
  • Form 48/Daily Time Record (DTR) – all copies should be original (for the applicable period)
  • Approved Form 6
  • S.O. maternity/personal leave
  • S.O. reinstatement
  • Certificate of Live Birth (for maternity)
  • Medical Certificate(CS Form 41)
  • Teacher’s Clearance

Documentary Requirements for DOUBLE PAY CLAIM DUE TO MATERNITY LEAVE:

In 1 folder with 2 copies each and all photocopies must be authenticated by the Administrative Officer.

  • Letter request with contact number
  • Special Order for Maternity Leave of Absence
  • Special Order for Reinstatement of Duty
  • Approved CSC Form No. 6 – Leave Application
  • Medical Certificate(CS Form 41)
  • Birth Certificate of Newborn
  • Latest Pay slip
  • Teacher’s Clearance

Documentary Requirements for UNUSED MATERNITY LEAVE (assumption before the expiration of the 60-day maternity leave)

In 1 folder with 2 copies each & all photocopies must be authenticated by the Administrative Officer.

  • Letter request with contact number
  • Form 7/Monthly Payroll Worksheet & Report of Service – all copies should be original (for the applicable period)
  • Form 48/Daily Time Record (DTR) – all copies should be original (for the applicable period)
  • Approved Form 6
  • S.O. maternity/personal leave
  • S.O. reinstatement
  • Certificate of Live Birth (for maternity)
  • Medical Certificate(CS Form 211)
  • Teacher’s Clearance

Documentary Requirements for LAST SALARY

In 1 folder with 2 copies each and all photocopies must be authenticated by the Administrative Officer.

  • Letter request with contact number
  • Form 7/Monthly Payroll Worksheet & Report of Service (for the applicable month)
  • Form 48/Daily Time Record (DTR) (for the applicable month)
  • Division Clearance
  • Certificate of Last Payment
  • S.O. for Retirement/Resignation
  • Last Payslip Received

Documentary Requirements for SALARY DUE TO HEIRS OF DECEASED EMPLOYEE

In 1 folder with 2 copies each and all photocopies must be authenticated by the Administrative Officer.

  • Letter request with contact number
  • Form 7/Monthly Payroll Worksheet & Report of Service (for the applicable month)
  • Form 48/Daily Time Record (DTR) (for the applicable month)
  • Division Clearance
  • Certificate of Last Payment
  • S.O. for Retirement/Resignation
  • Last Payslip Received
  • Death Certificate authenticated by NSO
  • Marriage Contract authenticated by NSO
  • Birth Certificates of surviving heirs authenticated by NSO
  • Designation of next-of-kin
  • Waiver of right of children 18 yrs old & above

Documentary Requirements for SALARY DIFFERENTIAL:

In 2 authenticated photocopies

  • Letter request with contact number
  • Notice of Salary Adjustment (NOSA)/Notice of Salary Increment (NOSI)/Appointment(in case of promotion)
  • Certificate of Assumption or First Day of Service
  • Pay slip of Last Unadjusted Salary
  • Pay slip of First Adjusted Salary
  • Certificate of No Leave of Absence Incurred Without Pay
  • Service Record (for claims on prior years)

Documentary Requirement for RATA

In 2 original copies

  • Approved DTR

Documentary Requirement for Clothing Allowance (Individual Claim)

In 2 authenticated photocopies

  • Appointment
  • Certificate of Assumption to Duty
  • Certificate of non-payment from previous agency, for transferees

Documentary Requirement for Subsistence, Laundry and Hazard Duty Pay for Health Personnel

In 2 authenticated photocopies

  • Approved DTR
  • Authority to collect (for initial claim)
  • Travel Report
  • Special Order from the Head of Agency

Documentary Requirements for SALARY INTEGRATION TO THE REGIONAL PAYROLL:

All photocopies must be authenticated by the Administrative Officer.

  • Letter request with contact number
  • Attested/Unattested Appointment (photocopy)
  • Certificate of Assumption to Duty
  • BIR Form 1902/2305 (with Stamp from BIR)
  • DBP ATM Account Number (Deposit slip of Initial Deposit)
  • Pag-ibig Number (Membership Form)
  • Philhealth Member Data Record (MDR)
  • GSIS Member’s Information Sheet
  • Certificate of Last Payment (for transferees)

Documentary Requirements for SALARY ADJUSTMENT DUE TO PROMOTION

1 copy each only & all photocopies must be authenticated by the Administrative Officer

  • Indorsement
  • Letter Request with contact number
  • Photocopy of Attested Appointment
  • Adjusment Form (all original/downloadable from the website)
  • Photocopy of latest payslip

Documentary Requirements for SALARY ADJUSTMENT DUE TO SALARY DEDUCTIONS & OTHERS

1 copy each only & all photocopies must be authenticated by the Administrative Officer.

  • Indorsement
  • Letter Intent with contact number
  • Adjusment Form (original/downloadable from the website)
  • Loan details which show monthly amortization
  • Photocopy of latest payslip

Documentary Requirements for SALARY ADJUSTMENT DUE TO STOPPAGE OF DEDUCTIONS

1 copy each only & all photocopies must be authenticated by the Administrative Officer

  • Indorsement
  • Letter Intent with contact number
  • Adjusment Form (original/downloadable from the website)
  • Original Certificate of Full Payment
  • Photocopy of latest payslip

  1. I miscarried last april 15, 2020. So my 60 days maternity leave fall on summer vacation. Double pay pa rin ba pag miscarriage?
