End of the Year Requirements for Principals

Republic Act 9155 otherwise known as “Governance of Basic Education Act of 2001” specifies the role of a school head both as an instructional leader and administrative manager. The school head has the authority, accountability and responsibility for administering and managing all personnel, physical and fiscal resources of the school.

In this regard, the following End of the Year Reports are required for submission to the Division Office:

End of the Year Reports

  1. Office Performance Commitment and Review Form (OPCRF)
  2. Summarized Report on Performance of Teachers/Non-Teaching Personnel
  3. Annual Implementation Plan
  4. School Report Card (SRC)
  5. Year-End Summarized Financial Report on School Canteen
  6. Summary of Annual Physical Examination of Teachers (CSC Memorandum Circular No. 17, s. 1989)
  7. School Improvement Plan (2018-2021)
  8. Annual Inventory
  • City School Board Properties
  • FI.E. Properties
  • National Property (IMC)
  • Principal’s Office
  • Classroom Properties
  • Donated Properties
