TEACHERPH is hereby reiterating the basic requirement for the application of Long Leave of Absence (with or without pay) to wit:
Table of Contents
Long Leave of Absence (15 days and above)
A. Without pay
- Intent letter to leave (indicate specific reason)
- Endorsement of the School Head
- Duly Accomplished CS Form 6
- School Clearance
- Duly accomplished Division Clearance for more than one month (CSC Memorandum Circular No. 41 s. 1996)
- Duly accomplished CS form 41/Medical Certificate (if sick leave)
B. With pay
- Intent letter to leave (indicate specific reason)
- Endorsement of the School Head
- Duly Accomplished CS Form 6
- School Clearance
- Duly accomplished CS form 41/Medical Certificate (if sick leave, maternity leave and Magna Carta)
- Incident /Police Report and medical results for availing of rehabilitation leave (CSC-DBM Joint Circular no. 01 s. 2006 Guidelines for availing of Rehabilitation privilege)

Nakaleave po ako due to my health condition. Wala po ako sweldo, bkt po kakaltasan pa dn po pag pumasok? lalo na pong naghirap, wala na nga po maibili ng gamot dhil walang sweldo. ok lng nman po kung wala sweldo pero sana nman po wag na kaltasan pag pumasok.

Up to how many years ( maximum) we can avail the long leave of absence.Is it possible yung 3 years
Good afternoon po.. valid po kayang reason for leaving (6mos) ang walang bakuna walang vaxcard kase di pedeng bakunahan? Total ayaw naman kaseng papasukin ang hindi bakunado edi mag leave na lng. Salamat po sa pedeng mkasasagut.

Good day po. Ako po ay cancer patient breast cancer stage 3 po undergo chemotherapy .Last chemo ko na po ngayong Feb 28 pang 8 session na po. Hello, I would like to ask if I would be allowed to apply for Long leave of Absence without pay for 6 months after 6 months of employment for me to have time for review before the boards? Not LET but a different board examination, Engineering Board Examination?
