  7. Oath of Office – original + doc. stamp
  8. Statement of Assets, Liabilities and Net Worth (SALN) Form
  9. BIR Form 1902 if applying for TIN or Form 2305 if already with TIN duly received by BIR agent
  10. GSIS (Membership Information Sheet) duly received by Remittance section
  11. Plantilla
  12. Transmittal of Appointments to be acted upon by CSC
