DepEd End of the Year Requirements for Principals

Republic Act 9155 otherwise known as “Governance of Basic Education Act of 2001” specifies the role of a school head both as an instructional leader and administrative manager. The school head has the authority, accountability, and responsibility for administering and managing all personnel, physical and fiscal resources of the school.


The following End of the Year Reports are required for submission to the Division Office:

Office Performance Commitment and Review Form (OPCRF)
Summarized Report on Performance of Teachers/Non-Teaching Personnel
Annual Implementation Plan
School Report Card (SRC)
Year-End Summarized Financial Report on School Canteen
Annual Inventory

Classroom Properties
Donated Properties
Summary of Annual Physical Examination of Teachers (CSC Memorandum Circular No. 17, s. 1989)
