PRC Steps in Filing Application for Registration Without Examination

Steps in Filing Application for Registration Without Examination

  1. Fill-out application form together with required documents for pre-evaluation to the Office of the Assistant Secretary (3rd Flr., Main Bldg.) at the PRC Central Office and Regional Offices processing counters.
  2. Go to the cashier for payment of fees. (Ground Flr., Main Bldg.)
  3. Go to the Customer Service Center for metered documentary stamp and notarization. (Ground Flr., Annex Bldg.)
  4. Submit duly accomplish application form to the Office of the Asst. Secretary.
  5. After three (3) months, verify status of application at Tel. No. (02) 735-1533.

CHEMICAL TECHNICIAN

  1. Original and photocopy of Birth Certificate (NSO)
  2. Original and photocopy of Marriage Certificate NSO (for married female)
  3. Original and photocopy of Transcript of Records
  4. Original and photocopy of College diploma/High School diploma
  5. Duly notarized Service Record
  6. Duly notarized Certificate of Employment
  7. Certificate of Experience (PRC Form 104)
  8. NBI Clearance

CHEMIST

  1. Original and photocopy of Birth Certificate (NSO)
  2. Original and photocopy of Marriage Certificate NSO (for married female)
  3. Original and photocopy of Transcript of Records
  4. NBI Clearance
  5. Original and photocopy Doctoral Degree Diploma
  6. Copy of Thesis

DETAILMAN

  1. Original and photocopy of Transcript of Records/Diploma

FISHERIES TECHNOLOGIST

  1. Original and photocopy of Birth Certificate (NSO) issued by NSO on Security Paper.
  2. Original and photocopy of Marriage Contract issued by NSO on Security Paper (for married female).
  3. Original and photocopy of Transcript of Records with Special Order (B), for graduates in private schools. If doctorate degree is obtained abroad, a certification stating that the college/university has been recognized or authorized to operate. (BS, MS, Ph. D.) & Diploma.
  4. Certified Copy of report of rating issued by the Civil Service Commission showing that the applicant has passed a civil service examination in Fisheries or the Career Service Professional examination.
  5. If employed in the Government, original and certified copy of service record, duly notarized.
    If employed in private sector, a certificate under oath issued by the authorized official/officer of the firm, company, association or corporation engaged in the agriculture industry showing that the applicant has served for at least five (5) years in a position which requires baccalaureate degree in Agriculture or the Career Service Professional eligibility; or
    (with detailed job description)
  6. Certification under oath of the authorized official/officer of a private school, college/university showing that the applicant has served for at least 5 years as faculty member/instructor and has taught subjects in Agriculture.
  7. Original and updated NBI Clearance (Private Employee).
  8. Original and updated Ombudsman Clearance and NBI Clearance (government employees).
  9. 3 Certification of Good Moral Character (barangay, church, school or employer / dean).

PROFESSIONAL ELECTRONICS ENGINEER

  1. Valid PRC ID as Electronics Engineer or Certificate as Electronics Engineer
  2. Valid IECEP ID or Certificate of Membership in good standing from IECEP
  3. Certified experience record of active self-practice and/or employment either in gov’t. service or in private sector, in the format to be prescribed by the Board, indicating the inclusive dates, companies worked for, description of specific responsibilities, relevant accomplishments and name, position of immediate supervisor for a period of at least seven (7) years (inclusive and/or aggregate), at least two(2) years of which are in responsible charge of significant engineering work, from the date applicant took his/her oath as and Electronics and Communications Engineers or Electronics Engineer.
  4. Three (3) certification signed by three (3) PECE attesting that the experience record submitted by the applicant is factual (not needed for those who have been registered and licensed as ECE under R.A. 5734, for at least seven (7) years.
  5. Original NBI Clearance (Private Sector)
  6. Original Ombudsman Clearance (for those in gov’t service)

PROFESSIONAL ELECTRICAL ENGINEER

  1. Original and photocopy of Transcript of Records
  2. Original and photocopy of Birth Certificate (NSO)
  3. Original and photocopy of Marriage Certificate NSO (for married female)
  4. NBI Clearance
  5. Valid certificate of registration and Professional ID as REE
  6. Certificate of Experience (PRC Form 104)

MEDICAL LABORATORY TECHNICIAN

(flunkers with ratings of 70% or above)

  1. Report of rating (optional)
  2. Original NBI Clearance

METALLURGICAL PLANT FOREMAN

PROFESSIONAL ELECTRICAL ENGINEER

    1. Submit a proposed title & outline to the Board of EE for approval as to subject matter, organization, technical content and presentation. If disapproved, the Board shall advise the applicant citing the reasons therefore. If approved, please submit the following requirements:
      1. Proof of Philippine citizenship; e.g. Certificate of Naturalization, voter’s affidavit/ID card, Philippine passport/visa.
      2. Certificate of Live Birth and/or marriage contract (if applicant is a married female).
      3. NBI clearance (original)
      4. Valid certificate of Registration and a valid Professional license (card) as a Registered Electrical Engineer (REE).
      5. Certificate of Experience-PRC form 104 (revised Aug.’95) duly accomplished and certified under oath. The service record shall be reckoned from the date the applicant took oath as an REE (Asst. EE or Asso. EE)
      6. Technical Report/Thesis/Dissertation Paper…
      1. The following elements of the report shall be included in the order given below:

        • Title Page
        • Letter of Transmittal
        • Preface
        • Table of Contents
        • Lists of table, charts, drawings, etc.
        • Summary
        • Conclusions/Recommendations
        • Bibliography/References
        • Affidavit of sole Authorship
        • Affidavit of PEE 1
        • Affidavit of PEE 2
        • Affidavit of PEE 3
        • Curriculum Vitae (with ID picture)
        • Transcript of Record

”Avoid unnecessary and irrelevant datas including charts, drawings, pictures and information not referred to in the discussion.”

      1. Affidavit signed by three licensed PEE. The Board may call the vouching PEE for an interview in connection with the affidavit executed in favor of the applicant.
      2. Brief description of the most significant and outstanding achievement/contribution of the applicant in the field of electrical engineering.
  1. After approval of the proposed title and outline by the Board, submit all requirements for processing together with the five (5) copies of the Technical Report (TER). Four (4) which will be retained by the Commission and one (1) for the applicant.
  2. Payment of P600.00 for the examination fee.

ANY APPLICANT WHO FAILED IN THE ORAL EXAMINATION/INTERVIEW SHALL NOT RE-APPLY UNTIL AFTER AT LEAST ONE (1) YEAR FROM THE DATE OF NOTICE OF FAILURE. IN THE RE-APPLICATION, RULE 14 SHALL BE OBSERVED.

PROFESSIONAL MECHANICAL ENGINEER

  1. Original and photocopy of Birth Certificate of NSO-Issued Birth Certificate in NSO security paper.
  2. Original and photocopy of NSO-Issued Marriage Certificate in NSO security paper (for married female)
  3. Lists of designs undertaken with the supervision of a PME
  4. Curriculum vitae
  5. Two (2) titles of proposed engineering report each with a short write-up and table of contents
  6. NBI Clearance
  7. Updated Professional ID as Mechanical Engineer
  8. Affidavit of Competency – 2 copies
  9. Affidavit of Applicant
    • Certifying Officer should be a licensed Professional Mechanical Engineer. Please provide an updated copy of license.
  10. Affidavit of Applicant
  11. Certificate of Experience (PRC Form 104) duly notarized signed by a registered PME.
  12. Detailed Description of Machineries and Equipment Handled (PRC Form 105.20.1-A)
  13. Application Fee/Oral Examination Fee – Php 900 upon filing
  14. Self-stamped mailing envelope

RADIOLOGIC TECHNOLOGIST & X-RAY TECHNOLOGIST

  1. Original and photocopy of Birth Certificate (NSO)
  2. Original and photocopy of Marriage Contract in NSO (for married female)
  3. Original and photocopies of Transcript of Records and Diploma
  4. Certificate of Experience (PRC Form No. 104) – for Rad. Tech. only

  1. good day Mam /sir Clarification regarding the license for agriculturist with out board examination.I m a member of the Philippine National Police .I want to apply to get promoted.Because i have civil service eligibility but the level of my eligibility was only for second level position.Due to PNP standardization in 1998.My quiry is can i apply for Agriculturist without examination

    Can Civil service eligibility of brgy officials can be used for this matter?
