DIVISION GUIDELINES ON THE ISSUANCE OF CERTIFICATES

1. Attached are the Guidelines on the Issuance of Certificates to be observed in our Schools Division Office in order to:

a. recognize individuals and institutions who have contributed immensely to the success of the programs and projects of the Quezon City Schools Division;
b. protect the integrity of ail certificates issued by the Schools Division Superintendent, and
c. systematize the request and issuance of Certificates.

2. All certificates to be submitted to this Office shall follow the mechanics stipulated in the guidelines.

Division Guidelines on Issuance of Certificates

I. Rationale

The success of a project, event, or an activity is a product of collaborative efforts and support from individuals who in one way or another have played a vital role in the achievement of a certain goal. Without the cooperation of participants and partners, any institution cannot push forward to its desired destination.

A certificate in its various degree and form is considered as an effective tool to give due recognition to people or institutions who partnered with the Schools Division Office and made a significant impact to it. In view thereof, this Office issues this Memorandum to promote a system that can protect the integrity of the certificates issued for a particular purpose.

II. Objectives

  1. To recognize individuals and institutions who have contributed immensely to the success of the programs and projects of the Quezon City Schools Division;
  2. To protect the integrity of all certificates issued by the Schools Division Superintendent, and
  3. To systematize the request and issuance of Certificates.

III. Scope

These guidelines shall cover all programs/activities that need a certification from the Schools Division Superintendent.

IV. Mechanics

a. Kinds of Certification. The following are the different kinds of certification that can be signed/issued by the Office of the Schools Division Superintendent:

Certificate of Recognition

This certificate is given to individuals or institutions who brought honors, have introduced new solution/intervention to an existing problem, and have sponsored/donated something of very important value to the Schools Division Office and Quezon City public schools.

Certificate of Appreciation

This certificate is given to private individuals or institutions who have shared their time, talent or resources for the benefit of the Division. This may also be given to Schools Division officials/personnel who have contributed the same over and above their actual duties and responsibilities for the advancement of the goals of the Schools Division.

Certificate of Participation/Attendance

This certificate is given to individuals or institutions who have attended or participated in the programs/projects and events initiated by the Schools Division Office.

b. Signing of Certificates. All Certificates submitted for signing of the Schools Division Superintendent must be properly endorsed by personnel concerned/in-charge of the program/project/activity. The preparation and printing of certificates shall be the responsibility of the organizer.

c. Control Number. All certificates to be issued/signed by the Schools Division Superintendent shall bear a CONTROL NUMBER to be facilitated by the School Governance and Operations Division (SGOD). The SGOD reserves the right to deny the issuance of control number if the certificate does not conform with this Memorandum.

d. Certificates Obtained from other Institutions. Certificates obtained from other institutions must be DepEd-related and can be submitted to SGOD for authentication and issuance of control number provided that they present the following: original and photocopy of the certificate, certificate of appearance signed by the organizer, and invitation or memorandum that specifies the name of the personnel.

V. Transitory Provisions. All certificates issued by the Schools Division Office prior to the issuance of these guidelines shall lost its validity after one (1) year from the issuance of this Memorandum.

VI. Effectivity

These guidelines shall immediately take effect upon approval.
