DepEd Quezon City 2016 Paper Evaluation of Principals

MEMORANDUM TO:

For the information and guidance of the field the Elementary Division Personnel Selection Board will conduct the screening and paper evaluation of Principals on January 20, 2016:

Interested applicants will submit the following required documents (Original and Photocopies) to the Head, Personnel Services Section for authentication on or before January 18, 2016.

  1. Personal Data Sheet (CS Form 212)
  2. Transcript of Records
  3. Updated Service Record
  4. Performance Rating for the last three rating periods (SY 2012-2013, 2013-2014, 2014-2015)
  5. Certificate of Outstanding Accomplishments (Original and Photocopies)
  6. Certificate of Seminars/Training’s attended (Original and Photocopies)

The interview, screening and evaluation of pertinent documents will be held at the Quezon City Science Interactive Center, this Division from 8:00 a.m. to 4:00 p.m.

School Principal I – January 6, 2016
School Principal II – January 20, 2016 8:00 o 12:00pm
School Principal III-IV – January 20, 2016 1:00 to 4:00pm

Failure to submit the required pertinent documents on January 18, 2016 will mean forfeiture of one’s right to the position.

Immediate and wide dissemination of this memorandum is desired.
