2019 Required Documents for Special Hardship Claims

Teachers are entitled to a Special Hardship Allowance pursuant to DepED Special Provision No. 1 and Republic Act No. 9401 otherwise known as “2007 General Appropriations Act” following the stipulations provided under the National Budget Circular No. 514, s. 2007 of the Department of Budget and Management (DBM) entitled “Provision and Computation of Special Hardship Allowance”.

To ensure the timely compensation of qualified teachers, submission of the supporting documents for the Special Hardship Claims shall be done QUARTERLY.

Pre-requisite on the claims is the submission of the supporting documents before the Accounting Section of your Division Office.

2019 Required Documents for Special Hardship Claims

Please heed on the list of documents to be submitted as to its classification:

Distance

  • Form 48 (DTR)
  • Biometric record
  • Payslip (for claimants w/atm)
  • Certification from Barangay Captain and PSDS
  • Accomplishment Report

Multigrade

  • School Form 4
  • Form 48 (DTR)
  • Biometric record
  • Accomplishment Report
  • Payslip (for claimants w/atm)

ALS Mobile Teacher

  • Certification by the Education Program Specialist as to the number of Grade level handle and to be verified by CID head.
  • List of learners
  • Form 48 (DTR)
  • Payslip (for claimants w/atm)
