For complaints, it is hereby reiterated that Section 5. Content of Complaint, DepEd Order No. 49, s. 2006 as quoted below shall be followed:
“Section 5. Content of Complaint – A complaint shall contain the following:
a. Full name and address of the complainant
b. Full name and address of the person complained as well as his position and office in the Department of Education
c. A narration of the relevant and material facts which should show the acts or omissions as allegedly committed by the person
d. Certified true copies of documentary evidence and affidavits of his witnesses if any; and
e. Certification or statement on non-forum shopping”
The conduct of clarificatory hearings would depend on the gravity of the offense, and pieces of evidence submitted, taking into consideration the health and safety of the investigators and persons complained of and the person filing the said case (COVID-19) pandemic.
While awaiting further guidelines from the region or central office for administrative formal hearings, those cases filed wherein the person complained of has been notified and given ample time to file his/her Answer already, the submission of factfinding/preliminary investigation report is on the discretion of the Investigator/s whether or not to conduct further clarificatory hearings.

Who May Avail of the Service?

What are the Requirements?
Complaint

Good day, Ask ko lang po paano makefile ng compliant against sa Professor ko nawalang consideration sa mga student nya alam nya online class Hindi lahat updated tulad ko pamilyadong tao full-time sa work. Nawala kasi cp ko kaya nawala contacts ko pati gc san nagaannounce Prof ko so, usually kapag exam nagnonotify naman admin kailan exam sa gmail ko kaya nakikita ko gamit cp ng hubby ko at usually final exam i-upload ang thesis then saka magdedefense kasi ichecheck pa daw. Then, final exam na nagulat ako close na exam sa kanya. Nakikiusap bat close na ito yung araw ng exam. Never man lang nagnotify yung Prof ko sa Lms alam naman online class at hindi mandatory yung pagsali sa zoom plus nawala cp ko kaya wala ako access sa gc namin sa viber. Now, kahit anu paliwanag ko ayaw ako pagbigyan. Unang-una sila ang mali kasi dapat practicum lang subject ko pero sabi nila pwede ko isabay thesis 2 ko. Tapos biglang nag-announce sila na hindi pwede pagsabayin, so sa batch namin ako lang kumuha ng thesis 2 at nagenrolled. Now, wala man lang konsiderasyon na makapagdefense. Magisa ako sa batch namin.
Hi, pwede po ba icomplain ung head teacher na laging nagpapagawa na hindi related sa school, like example ung assignment po ng anak nya, online birth certificate nya, online passport appointment. Aside pa po don, lagi po pinapagawa ung report nya sakin as head teacher, example po ung MPS Consolidation, ung MEMOs, Accomplishment Report Card ng Department namin, na nag ccause npo ng inconvenience sa role ko as as teacher, hindi lang po yon, ung isang kasamahan ko po, ginawa n nyang personal alalay nya, na pati foods sya ang bumibili at ipinapahatid pa nya food ng anak nya.
Paano po makakapag-file ng complaint sa head teacher na nag-post ng mga below the belt na salita at nagsabi pa na umalis sa DepEd? Personal attack na po kasi ang naramdaman ko sa mga sinabi niya at hindi ko na kakayaning makatrabaho ang ganung klase ng head teacher na imbes na mag-motivate sa tao niya, ay siya pa ang dahilan at nagtutulak sa guro na mag-resign. Noon ding nagpapapirma ako ng clearance, ayaw niyang pirmahan dahil gusto niya iwan ko yung mga personal kong ginawa na video lessons, PPT presentations ng lessons, Word file ng mga dokumento gaya ng WHLP para di na gagawa yung mga teachers niya at madali nilang mabubura yung pangalan ko sa “Prepared by”. Inupload ko naman sa Google Drive lahat ng mga soft copy ng IMs at iba pang files ko pero dahil ang sumbong sa kanya ng mga “assistants” niya na PDF files daw ang inupload ko kaya di nila maeedit (wala akong inupload na PDF file maliban sa mga na-download ko na soft copy ng modules na galing sa DO), di daw niya pipirmahan ang clearance ko at sinabihan pa akong “pabigat” at “wala sa ayos”.
